Clinical Skills Assessment IV (CSA IV)

The dates for the 2008 CSA IV are August 12, 13, 14, 18, 19, 26, 27 and 28, and October 3 and 24.

How to Change Your Scheduled CSA IV Date

To discuss changing your CSA date, please contact Jinny Fedorchak, Administrative Coordinator, at 330.325.6750.

Your request must be in writing, with your signature.

Faxed copies are acceptable. The Wasson Center fax number is 330.325.5917.

Your originally scheduled date
The date change that you are requesting
Your signature
Your telephone number and e-mail address

CSA IV Remediation

The date for CSA IV Remediation is October 24, 2008. This date cannot be changed.
