2007-2008 Campus Based Loan Forms

Do not complete any loan forms unless the loan has been offered to you by the Office of Student Services.

• NEOUCOM Campus Based Aid Information Sheet
• NEOUCOM Foundation Loan (This type of aid is offered to
  third- and fourth-year students - AMA-ERF, Davis/Price, Drach, Physicians, Rogers, Strouss, Taggart, Tschantz, Trumbull)
• Loan for Disadvantaged Students
• Perkins Loan
• Primary Care Loan

Authorization to Apply Federal Financial Aid to Miscellaneous Non-Allowable Charges, Prior Year Charges and Future Terms.

• In order to have your Federal Aid pay for your student insurance, other miscellaneous non-tuition related charges, prior year charges or future terms the federal government requires that you authorize NEOUCOM to do so by signing the form we have created below.

  Please return the completed form to Lynn Dreher in the Student Accounts area of the Accounting Department.

Emergency Loan for Medical Students

Please contact Lynn Dreher, Specialist Student Accounts, ext. 6373 to see if you qualify for an Emergency Loan before completing the application.

This loan is offered in increments of $300.00 or $600.00. A $1.00 finance charge is assessed to every $100.00 borrowed. The entire loan amount plus finance charges will automatically be deducted from any financial aid or payment placed on the student's account thereafter.

All approved emergency loan applications received before 3:00 p.m. on Thursday will be processed by noon on Firday.

You must also complete the above mentioned Authorization to Apply Federal Financial Aid to Non-Allowable Charges form in order for this loan to be taken out of your next finanical aid disbursement.

Please return the completed Promissory Note to Lynn Dreher in the Accounting Department. If your Emergency Loan paperwork is received before 4:00 p.m. on Thursday, your check will be processed Friday morning and mailed to your local address unless otherwise noted.  If your paperwork is received after 4:00 p.m. on Thursday, your check will be processed the following Friday.
