ELIGIBILITY FOR HEALTH INSURANCE PROGRAMS: Employees must be full-time and work 40 hours per week.
EFFECTIVE DATE OF COVERAGE: First date of employment
ENROLLMENT: Employees must complete the health benefits insurance enrollment process within 30 calendar days of employment to be covered. If you do not enroll during this time period, you will default to the PPO through Medical Mutual with employee only coverage. The exception to the time sensitive enrollment restriction is when an employee experiences a qualifying life event (QLE). Some examples of a QLE include getting married or divorced, having a baby or adoption, leaving for active military service or retiring. When one of these events occurs in your life, you have a short window of opportunity to alter your level of participation in our benefit programs. This window of time allows you to complete and submit the appropriate support documents to the Human Resources Department.
