LONG-TERM DISABILITY INSURANCE

Carrier:  UNITED HEALTH CARE

ELIGIBILITY:  Same as Medical Coverage

EFFECTIVE DATE:  Same as Medical Coverage

DESCRIPTION:  Our program offers four options:  60% after 6 months (college provided), 60% after 3 months, 70% after 6 months and 70% after 3 months.  The benefit maximum is limited to $60,000 per year.  If your salary is greater than $85,715 per year, there is no advantage to electing a 70% of salary benefit.  An employee must exhaust all their paid leave time (sick, vacation or compensatory) before payments begin.

COST:  Employer pays for 60% after 6 months coverage; employee may pay the difference in cost if they choose to buy up.

 

SHORT-TERM DISABILITY INSURANCE

Carrier:  FORT DEARBORN LIFE

ELIGIBILITY:  Same as Medical Coverage

EFFECTIVE DATE:  Same as Medical Coverage

DESCRIPTION:  This program provides protection from total loss of salary in the event an employee is temporarily disabled because of illness or off-the-job injury.  Benefits begin 15 days after an accident or 29 days after a medical emergency. 

Employees must exhaust all their sick leave balance before benefits begin.  They are not required to use their vacation or compensatory time.  The protection offered by this coverage is 50% of an employee’s wage or salary, to a maximum of $1,200 per week.

Short-term disability has a "pre-existing conditions" exclusion.  A pre-existing condition means a sickness or injury for which you have received treatment within 12 months before your effective date.  Any disability contributed to or caused by a pre-existing condition within the first 12 months of your effective date will not be covered.

COST:  Premiums are 100% paid by employee and are taken on an after-tax basis.
