DESCRIPTION:  This is a term insurance policy offering basic Life/Accidental Death and Dismemberment coverage to the employee.  This coverage is equal to your annual base salary rounded up to the next higher $1,000 and doubled with a maximum coverage level of $100,000.  Basic Life/AD&D is provided to all eligible staff at no cost.  Other options an employee may elect at no cost are $10,000, $50,000, 1X Annual Earnings (up to a maximum of $100,000), 1.5X Annual Earnings (up to a maximum of $100,000).  An employee may elect to waive the coverage.

COST:  Employer pays entire premium.

 

SUPPLEMENTAL LIFE INSURANCE

ELIGIBILITY:  Same as Medical Coverage

EFFECTIVE DATE:  Same as Medical Coverage

DESCRIPTION:  This is additional term life insurance.  An employee may elect to increase their coverage to 1X Annual Earnings (if over $100,000) 2X Annual Earnings (if over $100,000), 3X Annual Earnings, 4X Annual Earnings, or 5X Annual Earnings. Please note the maximum coverage for Life Insurance is $1,000,000.  Any coverage over $500,000 will require a physical.

COST:  Employee pays any premium over the College Provided Insurance.

 

Dependent Life Insurance

ELIGIBILITY:  Same as Medical Coverage

EFFECTIVE DATE:  Same as Medical Coverage

DESCRIPTION:  Our program also offers Dependent Life Insurance for your spouse and/or eligible dependents.  You may cover your spouse only or a child only or both.  Employee pays entire premium.  Two tiers are available:

$10,000 for your spouse; $5,000 for each dependent child        Cost :  $33.00 annually

$5,000 for your spouse; $2,500 for each dependent child          Cost :  $16.56 annually
