Submit FAFSA
- To apply online, you must have a Personal Identification Number (PIN). It takes three days to receive it electronically or seven to ten days to receive it in the U.S. Mail.
- Enter School Code: G24544. This code is to be used by both medical and pharmacy applicants.
- DO NOT leave any fields blank.
- DO NOT use estimates. Complete the IRS 1040 tax form before completing the FAFSA.
- Complete ONLY the student section if you wish to ONLY be considered for federal Stafford loans. Complete the student and parent sections if you wish to be considered for federal Stafford loans and Campus-based aid (institutional grants, institutional loans, Scholarship for Disadvantaged Students, Primary Care Loan, Loans for Disadvantaged Students, Federal Perkins Loan).

Statement of Certification - this form allows us to release financial aid and tuition billing information to anyone you specify on this form. (All students) |

Campus-Based Aid Application*- only to be submitted by students who wish to be considered for both federal Stafford loans and Campus-based aid (must provide parent information on the FAFSA to be considered). |

Authorization Form - this form allows us to apply federal financial aid to both your tuition and fees. If this form is not submitted, your financial aid will be applied to your tuition; however, you will have an outstanding balance due to the College for your fees that you will need to pay. (All students) |

| *Providing parental data on the FAFSA and Campus-based Aid Application will NOT make you "dependent." It is the College's policy that all students must provide parental information in order to be considered for campus-based aid, and the policy of the Department of Health and Human Services for some types of federal aid. All students are encouraged to provide parental data in order to take advantage of all financial aid opportunities. Awards will be available on DOCS by mid-June. At that time, students may apply for loan funds.
